Whether your concern is health for an opiate addiction, alcohol addiction, methadone addiction or some other addiction, your health should be considered an asset in steps toward recovery. Your health should be your greatest asset.</p>
<p style="text-align: justify;">By putting drugs in your body you are making toxic investments in to your most valuable asset, your health. Think for a moment about the health issues that may arise from an addiction problem. You may develop any of the following as consequences of drug abuse or use: HIV, Hepatitis C, skin problems, liver problems, tooth decay, bone problems, venereal diseases and a myriad of other health issues.<span id="more-538"></span></p>
<p style="text-align: justify;">Your good health is an asset. Your health should be your greatest asset. Unfortunately, you have made toxic investments in your health. It is time to make your health a valuable asset.</p>
<p style="text-align: justify;">The investment in your health will not only increase your body function, hence increasing your asset, but will be profitable to you in your life style and maybe even in terms of money. God gave us a body that will function properly if it is adequately and properly fed nutritionally.</p>
<p style="text-align: justify;">If you have an opiate addiction, heroin addiction, drug withdrawals, methadone withdrawals or you are in methadone treatment, opiate treatment or any drug treatment you owe it to yourself to improve your health which is main your asset.</p>
<p style="text-align: justify;">As your body becomes an asset to you, you again become an asset to society. Invest today. Invest wisely. It&#8217;s your health. It&#8217;s your life. It is time for you to live your life to the fullest.</p>

<p>Physical therapy forms should cover the following aspects of patient-client management: initial examination/evaluation, visit, re-examination and discharge.<span id="more-241"></span></p>
<p style="text-align: justify;">Every visit by a patient should be supported by the relevant physical therapy forms. The patient&#8217;s full name and identification number should be documented on every form. All physical therapy forms related to examination, evaluation, visits and discharge must be authenticated by the physical therapist that provided the service. All physical therapy forms should mention the referral source through whom the patient was sent for the visit.</p>
<p style="text-align: justify;">Initial examination or evaluation forms should include the following data: general demographics, social history, employment details, growth and development, living environment, <a title="First Health" href="http://www.cours-prepa.info/first-health" target="_blank">general health</a> status, <a title="Health Definition" href="http://www.cours-prepa.info/health-definition" target="_blank">health</a> and social habits, family history, medical or surgical history, main complaints at present, current medications, and other <a title="Clinics Health" href="http://www.cours-prepa.info/clinics-health" target="_blank">clinical tests</a>.</p>
<p style="text-align: justify;">Forms related to visits should ask the patient for the following information: self-report by the patient, changes in patient condition and response to interventions.</p>
<p style="text-align: justify;">Discharge forms should contain the patient&#8217;s current physical status, degree of improvement, and referrals for additional services.</p>
<p style="text-align: justify;">If the therapy is covered under &#8216;No Fault <a title="Aetna Health" href="http://www.cours-prepa.info/aetna-health" target="_blank">Insurance</a>&#8216;, the patient will need to fill in a patient information form, appointment policy form, financial policy form, Assignment of Benefits form, history form and an authorization form.</p>
<p style="text-align: justify;">If covered under workman&#8217;s compensation, a patient would be required to fill in a patient information form, appointment policy, history and authorization.</p>
<p style="text-align: justify;">For a patient not covered by any of these insurance covers, the following forms would need to be filled out: patient information, <a title="Health Facts" href="http://www.cours-prepa.info/health-facts" target="_blank">health survey</a>, <a title="Health Line" href="http://www.cours-prepa.info/health-line" target="_blank">health questions</a>, <a title="Centers Health" href="http://www.cours-prepa.info/centers-health" target="_blank">financial policy</a>, appointment policy, history and authorization.</p>
